    Personnel Meeting
    S.Y. 2015 – 2016

    Date & Time : January 22,2016/ 3:00-4:00 p.m

    Agendum # 1: Class Observation Sheet from June to December
    Mrs. Adante informed the teachers that the class observation sheet from June to December
    will not be used anymore and we will use a new class observation sheet. She informed also that the
    teachers will meet the coordinator for the schedule of our class observation.
    Agendum # 2: Fun Run - Red Cross
    Fun run is scheduled on February 5,2016 held at Bislig City with a registration fee of 400
    pesos.
    Agendum # 3: Monitoring Incharge
    The monitoring incharge in Lingig District is Dr. Willie Dumpit.
    Agendum # 4: RPMS
    The RPMS is need to be evaluate.
    Agendum # 4: National Achievement Test
    The National Achievement Test will be held on March 2, 2016.
    Agendum # 5: Annual Medical Exam
    The annual medical exam is composed of urialysis,x-ray,dental,and pap smear.

    MINUTES OF THE THIRD HRPTA MEETING


    S.Y. 2015 – 2016

    Date & Time : November 26, 2015 / 8:00 a.m. – 10:00 a.m.
    Venue : Grade 7 – Virtuous

    Agendum # 1: Distribution of Cards

    As the class adviser of Grade 7 – Virtuous, I immediately gave the report cards to the
    parents of the students in attendance as we opened the meeting to order. The parents compare
    the grades of their child in 1st grading and 2nd grading if it increases or decreases, and after
    that they signed the cards and returned it back to me.

    Agendum # 2: Christmas Party

    Ms. Lolievelle L. Malubay, a representative of Grade 7 – Virtuous, informed the


    parents of what would be their plan for the upcoming Christmas Party. All I did was to get
    some suggestions and opinions from the parents for they are the one who will give financial
    support to the students for whatever contributions for the party would be.
    Mrs. Rige L. Doysabas suggested that the worth for the exchanging gift (things) is
    Php 50.00. Fortunately, all parents agreed to the suggestion of Mrs. Doysabas. Other parents
    suggested also that students will be grouped so that their group will decide of what food they
    will have to bring for the party, same as with the other group of students. With these, I
    promised them that I will let them know/send them a letter if there is already a grouping from
    the students together with the food they will have to provide for the party. I also informed
    them that as an adviser, I will also have to pledge also a dessert for the upcoming party.
    Lastly, I informed them about the attire to wear during Christmas party. All suggestions were
    unanimously agreed by everybody.

    Agendum # 3: Other Matters

     Some of the parents paid the HRPTA Project and Miscellaneous.


     Other parents’ follow-up the performance of their child in school.
     Some of them gave me the NSO and Form 137-E.
     There are 18 parents attended the meeting.
     The meeting was adjourned at 10:00 a.m.
