    -Ensures that 90% of deliveries in the assigned barangays will be
    referred to a birthing facility & will be attended by skilled health
    professionals
    1. Reduced the Maternal Mortality
    rate to less than 90/1000 Live Birth -Conducts home visits to households and 95% of all pregnant
    mothers will undergo 4 ANC & 2 postpartum visits
    -Participation of Barangay Health Committee through the BHB in
    maternal death review & infant mortality review will be advocated

    -All eligible children will be vaccinated and attain 95% FIC


    coverage through reaching every purok activities in assigned
    2. Reduced preventable Neonatal barangay
    Mortality to atleast 15/1000 Live
    Births and under Five Moratlity to -Provides advocacy to attain 70% of all infant EBF up to six
    22/1000 Live Births months
    -80% of all NIP protocols and updates in the assigned barangay
    will be implmented

    -Identification of clients with unmet Modern Family Planning in the


    assigned barangay so with outreach medical services will be
    conducted

    -Modern contraceptive method among WRA to atleast 65% in the


    3.Ensured universal access to sexual assigned barangay will be advocated
    reproductive health care services.
    -Strategies to decrease adolescent birth rate to 2% per year will
    be advocated
    -Assistance on Family Program services and the prevention of
    teenage pregnancy in the assigned barangay will be provided

    -100% advocacy that residents will be Evacuated to assigned


    4 .Reduction of Mortality rate due to
    Evacuation Areas and participate in data gathering response
    injuries
    during health emergencies and disaster.
