Pour stock and milk into pot; add a pinch of salt. Reduce heat to the lowest setting and
cook, uncovered and stirring occasionally, until meat is very, very tender, 2–2½ hours.
There shouldn’t be any rapid bubbles at this stage. Instead, the sauce should release the
occasional small bubble or two. When finished, the sauce should have the texture of and
look like a sloppy joe mixture. If the liquid reduces before the meat is completely tender,
add an extra ½ cup stock and continue cooking. Discard bay leaf. Taste sauce and adjust
seasoning with salt; keep warm.
Cook pasta in a large pot of boiling salted water. If using fresh pasta, cook about 3
minutes. If using dry, cook until very al dente, about 2 minutes less than package
directions.
Using tongs, transfer pasta to pot with sauce. Add 1 cup pasta cooking liquid and ½ cup
Parmesan. Increase heat to medium, bring to a simmer, and cook, tossing constantly, until
pasta is al dente and liquid is slightly thickened, about 2 minutes.
Transfer pasta to a platter and top with more Parmesan.
Do Ahead: Sauce can be made 4 days ahead. Cover and chill.
