There has been a recent increase in incidents involving lifting equipment on the
project, during past month. Several Hydras have overturned, loads have fallen, and
a crane has overturned resulting in a fatality. Operator error, equipment failure and
procedural non-compliance have been the cause of these incidents.
The systems established at site need to be re-evaluated to improve work practices
and compliance to avoid any similar incidents occurring in future.
Action Required
• Follow the required project procedure for any lifting activity conducted at site.
• For all lifts (below 20 ton) daily lifting log for the particular crane to be maintained
by the contractor.
• Daily lifting log should provide the basic information about weight of the material
and its lifting radius / boom angle for particular crane.
• Simple risk assessment/JSA to be done for each lift accounting for site conditions.
• All cranes must be fitted with load radius indicator with automatic safe load
indicator.
• Authorized Crane operators certified by Consultant and rigging group only to be
