Pre-Application Medical Examination

 A medical exam may be required for work permit exceeding 6 months.


 You may find a list of panel physicians and a list of documents you will need to submit at
http://www.cic.gc.ca/english/information/medical/medexams-temp.asp
 It is not mandatory to complete a medical examination prior to submission of your
application. However, doing so may result in faster processing. Medical results are valid for
one year. Do not undergo a subsequent medical examination if you had one in the last year.
 After completion of your medical examination, the panel physician will provide you with one
copy of your medical examination form. This form must be submitted together with your
Work Permit application.
 Submission of this form does not guarantee approval of your application. Do not make any
commitments until your work permit application has been approved. All costs for the medical
examination are payable by the applicant and non-refundable.

Application for a Work Permit - Checklist

 A visa officer will make a decision based on the documents you submit. Additional
documents will not be requested. Failure to submit relevant documents may result
in your application being returned, delayed or refused.
 Any documents not in English or French must be accompanied by a certified
translation. Please submit photocopies only of official documents, as no documents will be
returned to you once your application has been processed.
 False statements or submission of fraudulent documents will result in refusal of your
application. In addition, you may become inadmissible from entering Canada for five (5)
years.
 The submission of an application does not guarantee the issuance of a permit/visa. Do not
finalize travel plans until you have received a visa.

You must submit the following: √

Original, valid passport. Include all cancelled or expired passports, if applicable. Your
passport should be valid for the duration of work in Canada, as the validity of a work permit
cannot exceed the validity of the passport.

Additional Supporting Documents to be submitted: √

For all Work Permit applications:

Evidence that you meet the requirements of the job offered:

 Proof of Education: Certificates, Diplomas or Degrees including mark sheets;

 Proof of work experience: Copies of appointment letters and employment termination


letters from your current and previous employers and copies of salary pay slips; bank
statements. Letter(s) of reference must outline the duration and specific nature of
your employment, including your exact duties on the job. They should also include
contact name, address and phone number of contact-persons.

 Proof of your language proficiency: Test results to demonstrate ability in the language
of proposed work. The test must have been taken within the last 24 months.

Proof of personal funds, including income tax returns for the past two years, bank
statements or books from the past three months. Property documents or other proof of income
or investments.

If you have answered “yes” to any of the background information questions on the application
form, provide a completed and signed Schedule 1 form (Application for temporary Resident
Visa – IMM 5257B) and include it with your application.

Completed Military table: If you are or have been part of the military or police forces, fill
out the military table at the Visa Application Centre (VAC) or request a questionnaire by
sending an email to [email protected]

Copies of marriage certificate and children’s birth certificates for accompanying


dependents.
Copy of medical examination form IMM 1017 if you opted to undergo a medical examination
before submitting your application.

Family Information Form (IMM 5707) fully completed, signed and dated.
