Each item found in violation is reported below along with the code requirement.
Inspector Findings: Open foods throughout facility contaminated or potentially contaminated by worker using
intervenous drugs in the kitchen. Correction: Owner discarded all open food items throughout
FSE. *Note: Discussed proper sanitizing procedures.
Code Requirements: Toxic chemicals are improperly labeled, stored or used so that contamination of food can occur.
Inspector Findings: 1. Observed via photos from Fire Marshall blood and hypodermic needles, from inter venous drug
use, on pizza prep table next to where dough was being prepared. Correction: Owner voluntarily
discarded all dough and all other potentially affected food items in FSE. *Note: Discussed proper
sanitizing procedures.
2. Observed spray bottles of ant poison, stored on counter by cash box, directly next
3. Pesticides and other chemicals stored on dish rack along with clean dishes and were in direct
contact with the rack. Correction: Owner voluntarily discarded pesticides and other chemicals or
moved to a dedicated chemical storage area. *Note: Discussed proper procedures to sanitize all
affected dishes and dish rack.
4. Ant poison stored directly on canned food item in storage rack. Correction: Owner voluntarily
discarded pesticide. *Note: Discussed proper sanitizing of canned food items and that all
pesticides must be applied by a Certified Pesticide Applicator.

Inspector Findings: Blood on sink, mirror and walls in bathroom from intervenous drug use. Correction: all surfaces
must be cleaned and sanitized before reopening. *Note: Discussed proper sanitizing procedures.

Inspector Findings: 1. Broken floor tiles throughout kitchen (floor must be smooth and cleanable).
2. Peeling paint on ceilings near pizza ovens and near condiment station in dining room.
3. Dust buildup on wood shingles above counter tops in dining area.
4. Grease and accumulated food debris on walls above prep coolers in kitchen area.
5. Grease buildup on ceilings throughout FSE.
6. Peeling paint on plumbing pipes in corner by 3-bay sink.
Code Requirements: Lighting and ventilation inadequate, fixtures not shielded, dirty ventilation hoods, ductwork, filters,
exhaust fans
Inspector Findings: 1. Light shields throughout FSE are soiled with accumulated grease.
2. Missing vent covers in ceiling of kitchen area.
3. Accumulated debris inside light shields in dining room area.
4. Heavy grease buildup on hood above pizza oven.
5. Accumulated dust and grease on hood vents above fryers.
Code Requirements: Premises littered, unnecessary equipment and article present, living quarters no completely
separated for food service operations, live animals, birds and pets not excluded
