    AFFIDAVIT OF SUPPORT AND CONSENT


    We, SPOUSES JEMA M. MILLET and ARTEMIO C. MILLET, both
    of legal age, Filipino citizens, and a residents of McArthur Highway, Tarlac City,
    Philippines, after having been duly sworn to in accordance with law, hereby
    depose and state – THAT:
    1. That we are the parent of CHRIS JAYSON M. MILLET, born on 05
    January 2000 at Tarlac City;
    2. That we are giving our consent to our child to travel abroad to be
    accompanied by us;
    3. That we are paying for the expenses of their travel;
    4. That we are financially capable to provide for their accommodation,
    sustenance and support during their sojourn;
    5. That we guarantee that they will not violate any law of their destination
    country;
    6. That I am executing this affidavit to attest to the truth of the foregoing
    statements and for whatever legal intents and purposes it may serve.
