A time thread tear is an entity created during the fight with Kerapac, the bound.

In normal mode, the tear is not considered an NPC as they only appear during his Shatter mechanic, although it will unleash damaging energy which will harm the player as magic damage before despawning.

In hard mode, the tear is considered an NPC and will linger on the field even after the mechanic concludes, periodically damaging all players inside the arena with light typeless damage until the rift is closed.

In both modes, the tear spawns behind the player(s)' back. Players can prevent the tears from spawning by standing directly underneath Kerapac (centre of his 3x3 tiles) before he slams his staff to the ground. However, if an Echo of Kerapac ends up appearing (as a result of phasing Kerapac when he managed to summon a rift), the method will not work.
