1. PURPOSE
This procedure provides guidance to all workers involved in scaffolding activity works
under the Misc. Instrumentation PCR’s at RG Plant in Dukhan Fields
The purpose of this procedure is ensuring IMCO develops and effectively implements the
Company policies, systems and provisions for the use of fall arrest and body recovery
equipment.
This procedure is structured to minimize the risk of injury to its employees and
subcontractors.
2. OBJECTIVES
This procedure provides guidance to personnel involved in scaffolding works and shall
apply to all scaffold activities in RG Plant area.
3. SCOPE
The scope of this plan is to establish the individual activities to be performed for the
installation of scaffolding structure within the scope of work of GC 14113600 as per the
project works requirement.
This procedure should be applied in all scaffolding activities as per this contract no. GC
14113600 inside RG Plant within the following PCR’s;

IMCO ensure that all HSE requirements in below references are strictly followed.
5. DEFINITIONS
Scaffold Tag – An administrative tool, i.e. a plastic card displayed on the scaffold
structure, used to control and records the operational status of the
scaffold.
Safety Harness – a safety harness fitted with a restraining line of lanyard and attached to
an anchorage point, is to arrest safely a limited free fall by the wearer. It
should be fitted with a restraining line or incorporating a pulley system,
which pays out the line on demand and rewinds it automatically when not
required. In either case, in the event of fall, the anchorage locks on to the
anchorage line should be within one meter of the point at which the load
is first applied, thereby limiting the fall.
Safety Lanyard – a safety lanyard is a piece of rope with loops for attachment of as
safety belt or harness to an anchorage point. It should be made of
synthetic fiber rope and may incorporate a shock-absorbing device to
provide the necessary degree of shock-absorption. It must be limited in
length so that the safety belt or harness is restrained to within allowable
free fall distance.
6. PROCESS
Ensure that all scaffold work in Dukhan Fields operations areas is covered under the
Permit to Work System and a separate work permit shall be issued for erecting or
dismantling of each scaffold.
A work permit for the erecting of scaffold cannot be signed off as complete until the
scaffold has been inspected and approved for use by the Scaffolding Supervisor.
A work permit for the dismantling of scaffold cannot be signed off as complete until the
scaffold materials have been removed from the site to a dedicated storage area.
After a scaffold has been erected and approved for use, any planned work on the
scaffold structure that requires a work permit must be conducted on a separate work
permit.
6.1Scaffold Materials
All scaffolds components, parts and materials shall conform to British Standard BS
5973 (1993), and shall be:
Fall arrest devices must be used when scaffolders are exposed to a fall of
greater than 2 meters, and are not protected by standard handrails or an
appropriate enclosure.
All scaffolders shall wear body harness and lifelines when working at a height
above 2 meters.
6.3Erecting of Scaffolds

All scaffold structures should be built on a firm and level base, with the
use of base plates to ensure stability.
Pipe work or other equipment must not be used for the securing of the
scaffolds.
Diagonal bracing shall be provided on all sides at all levels of the
structure and bracing material shall not exceed 3 meters in length.
Diagonal bracing shall be fitted at an angle of 45 degrees and, where
possible, shall be assembled upwards from the base plate level.
Scaffold poles over 2.44 meters in length shall be handled by two
persons, to prevent the blind end from inadvertently striking personnel,
equipment or instrumentation.
Where scaffold cross-members are less than 2 meters above ground
level, the cross-member shall be tapped with highly visible hazard
warning tape.

6.4General Instruction
Wherever reasonably practicable, platforms with guard rail and toe boards should
be provided for work at heights above two meters. Where this is not reasonably
practicable and a person at work is exposed to a fall exceeding two meters, an
appropriate and properly anchored fall-arrest device must be worn.
7. SCAFFOLDING DESIGN
The access and working scaffold listed in Table – 1, with their construction criteria shall be
design by IMCO Scaffolding team in accordance with appropriate QP Scaffolding Works
for Dukhan Fields Doc. No. IP-OPS-030 Rev. A2. All other scaffold, and all scaffold which
are the subject of a specific user request outside the specified format of Table - , shall be
designed by IMCO Scaffolding Team to the requirements and approval of Client (QP).
8. RESPONSIBILITY
8.1Safety Officer
Shall ensure that the scaffolding activities in GC 14113600 project are as per
QP standards IP-OPS-030 Rev. A2.
Shall ensure that only qualified scaffolding personnel are involved in
scaffolding activities.
Shall ensure the scaffolding personnel are complying with Permit to Work
requirements.
Shall ensure that all scaffolding personnel under his authority fully comply
with this procedure and Permit to Work requirements.
Shall ensure that all scaffolding is inspected regularly and up-to-date records
are maintained.
Shall ensure that all scaffold erected is checked properly as per standards.
Shall ensure that “long term” scaffold structures are regularly audited and
report submitted to the Safety Officer at site.
Shall ensure that all damaged or defective materials are identified, tagged
and returned to the stockyard premises.
9. PRE-REQUISITES
Before initiating the scaffolding work activities at site, following pre-conditions will be
satisfied:
All scaffold work in GC 14113600 project is covered under the Permit to Work
System and a separate work permit shall be issued for erecting or dismantling of
each scaffold.
Ensure proper survey should be done at site by the competent person to check the
loose pockets, levelling and adequate space for scaffolding.
Ensure that estimated materials will be prepared as per requirement to avoid any
near miss at site.
Layout of longitudinal and across sectional line on the actual ground.
Scaffolding shall only be erected, dismantled or modified by qualified scaffolding
personnel in accordance with QP requirements (IP-OPS-030 Rev. A2)
Insure supported scaffolds consist of one or more platforms supported by the
outrigger beams, brackets, poles, legs, uprights, post, frames or similar rigid
support.
Insure guardrails or personal fall arrest system for fall prevention/protection is
required for workers on platforms 10 feet or higher.
Insure working platforms/decks must be planked close to the guardrails.
Insure planks are to be overlapped on a support at least 6 inches, but not more than
12 inches.
Insure legs, post frames, poles, uprights must be on base plates and mud sills, or a
firm foundation, and be plumb and braced.
Ensure to check for safe access. Do not use the cross braces as a ladder for access
or exit.
A work permit for the erecting of scaffold cannot be signed off as complete until the
scaffold has been inspected and approved for use by the scaffolding supervisor.
Scaffolding inspector has been signed in the scaffold tag after he makes sure that
scaffolding is safe to use.
A work permit for the dismantling of scaffold cannot be signed off as complete until
the scaffold materials has been removed from the site to a dedicated storage area.
After the scaffold has been erected and approved for use, any planned work on the
scaffold structure that requires a work permit must be conducted on a separate work
permit.
10.1 Planning
In order to satisfy the requirement of personnel intending to work from the working
platform, the scaffolder will need to liaise with those personnel, the maximum load
likely to be placed on the working platform, the area of working platform necessary
to carry out the work, positioning of access ladders and a comfortable working
height for the working platform will all need to be discussed. Any hazards involved
with the erection shall be identified at the planning stage and appropriate
precautions taken. Proximity to live electrical equipment or interface problems may
need to be given consideration.
All scaffolds shall be braced and tied to form a rigid stable structure. There are
minimum requirements for bracing and tying scaffold but these is a guide only to
the scaffolder. A priority concern of the scaffolder is to provide a safe working
platform free from movement. If this priority is not satisfied on completion of the
minimum requirements for bracing and tying, the scaffolder is bound to include
such extra ties and braces as in needed.
10.2.1 Braces
With the exception of tower and single bay scaffolds, face braces shall be
provided that are attached to the external row of standards of the scaffold
as close to the junction of the ledger and standard as is practicable and
extend to the full length and height of the scaffolding. Transverse braces
shall be provided that are attached diagonally in every lift joining the internal
and external standards at both ends of the scaffold and at intervals not
exceeding 25 meters. Tower and single bay scaffolding shall be provided
with braces diagonally in every face in every lift of the scaffold.
10.2.2 Ties
Ties shall be provided that are attached by 90 couplers to the outside and
inside standards or ledgers as close as possible to the junction of ledgers
and standards. They shall be located horizontally no further than one bay
from the ends and at intervals not exceeding 3 bays. They shall be located
vertically in the first lift and at intervals not exceeding 4 meters.
10.3 Guardrails
All working platform above a height of 2 meters or at any height less than 2 meters
where a hazard such as starter bars exists, shall be provided with double guard
rails and kickboards to all open edges.
The guard rails shall consist of a top rail erected 1 meter above the working
platform and a midrail erected midway between the top rail and the working
platform.
Kickboards shall extend a minimum of 200mm above the working platform and
shall be secured firmly to the standards. Puncheons may be fixed to the guard rails
to facilitate the securing of kickboards if necessary.

10.5 Dismantling
Prior to any dismantling of scaffolding taking place and during the dismantling
procedure, the following safety precautions shall be observed:
The Scaff tag is to be changed from “Complete” status to “Incomplete” status.
The area at ground level is to be barricaded with general purpose bunting and
a sign displayed stating “Danger – Men Working Above”
Use Scaffolding shock bag for lifting the materials from down to up & vice
versa.
All planks are to be turned to remove dust and dirt that may cause eye injuries.
Care shall be taken to ensure personnel are not working below when plank
cleaning is taking place. All horizontal tubes, once removed, shall be turned to
the vertical to remove grit and metal flakes from inside the tubes to avoid eye
injuries during lowering operation.
Lying of sole board horizontally on the ground along with longitudinal lines at a
spacing of 1.2 meters.
Placing of adjustable base plate/jack base on the sole board at a spacing of
1.2 meters.
Placing of standard pipe on each base plate. Alignment of pipes for true 90
degrees vertically.
Couple vertical pipes with horizontal bracing pipes at 300 mm above the base
plate with the help of dual couplers.
Similarly 2nd horizontal pipe will be braced at 300 mm below the top of the
stand pipe.
Fix across bracing at inclination of 35 degrees to 45 degrees with the help of
Sewell couplers.
After completion of frame work, stirrup heads/ U jack will placed, in the pipes,
at top for support system of wood runners.
Place of 100 mm x 100 mm x 4 meters long wood runners in the U jacks in
longitudinal direction.
Place of 75 x 75 x 4000 mm long wood runners on the 100 mm x 100 mm x 4
meters long wood runners in cross sectional direction for placing of ply board
as slab bottom.
Care will be taken for the beams with respect to more support by bracing of
one or more horizontal at top level.
A walk way platform of 1.2 meters wide will be made by extended horizontal
pipes, along with the periphery of slab.
Guard hand rails will be fixed on the external side to the walk way.
Toe boards will be fixed on the guard rails.
At 75 degrees angle monkey ladder will be fixed at both ends of walk way.
After the completed scaffold structure has been inspected and approved for
use, the scaffolding supervisor shall remove the “Do Not Use” tag and replace
it with an “Approved for Use” tag, affixed in a clearly visible location.
The objectives of this JHA is to ensure that the basic steps of a job or task
are determined and identified any existing or potential hazards are
documented to ensure suitable and sufficient controls are in place to enable
individual work or task to be performed safely and effectively at all QP
facilities under this Project.
One of the purposes of this JHA is to provide a method for the identification
of hazards and control of risk arising from the workplace and the introduction
of new workplace, processes and substances to the facility and the means
by which these risks shall be eliminated or minimized.
First Aid Facilities – IMCO shall ensure adequate first aid resources are
available at the jobsite.
Site Work Rules – IMCO shall ensure all activities are conducted
