The extreme dominion medallion is an upgraded version of the super dominion medallion that is unlocked by killing 1,600 monsters in rumble mode in the Dominion Tower. Once this condition is met, the super dominion medallion can be upgraded into the extreme dominion medallion for free by speaking to strange face. The 1,600 monsters do not have to all be killed in the same instance in order to unlock the extreme dominion medallion. They can be killed over many sessions. This can be checked by reading the Dominion marker.

The medallion offers a universal accuracy buff of +1% that works in both PvM and PvP. This percentage is of the total accuracy of items worn and is calculated after your attack rating has been worked out.

The medallion provides six teleports a day to the Dominion Tower and recharges daily.

When worn inside the tower, the medallion boosts Attack, Strength, Defence, Ranged and Magic levels by up to 10 and Necromancy by up to 13.

If the extreme dominion medallion is lost, players must pay 700,000 coins to the Strange face to obtain another one.

If you die in the Wilderness and opted in PvP, the killer is rewarded with 420,000 coins.
